How to navigate the referral process

NDIS

At Survivor Physiology we believe every rural/remote client deserves the very best health care treatment.

 

As allied health professionals we understand that each individual's situation and circumstances will require careful consideration and planning for those living with a disability.

Survivor Physiology sessions promote: 

​

  • A welcoming environment 

  • Fun and engaging sessions

  • Inclusiveness 

  • Motivational and confidence building

  • Aid in activities of daily living

​

If you or a loved one are living with a disability, you may be eligible to access funding under the National Disability Insurance Scheme.

Step 1

Know your plan

If you are Plan Managed or Self Managed you  

You will need to check if you have sufficient funds allocated to the following categories: 

​

For Exercise Physiology services you will need to have Capacity Building (CB) funding under “Improved Health & Wellbeing” & “Improved Daily Living Skills”. or core supports. 

​

Please note Survivor Physiology does not currently support funding for those that are "NDIA managed".
